International Enrolment Form

International Enrolment

There are three ways to submit your enrollment form. Please contact the school office if you need assistance in completing the enrolment form.

Option 1: Printable Form

  1. Download and print the form linked below.
  2. Fill the form.
  3. Scan the form and email to international@lac.school.nz or post the filled form to 100 Walkers Road, Longburn, Palmerston North, New Zealand 4477.

Please note:

  • If your child is going to reside in the LAC House dormitories, then there is no need to fill our Part Four of the document, ‘Designated Caregiver Agreement’ .
  • For parent and student, please initial each page to acknowledge your understanding of the form and contract of enrolment.
  • Please include current school report(s) along with your application.
